Care Management Navigator is responsible for ensuring effective coordination of healthcare services from both the acute and post-acute care settings, bridging care and communication with the patient's primary care physician post-discharge and actively engaging the patient in overall healthcare management.
The role of the Care Management Navigator is crucial to increasing patient engagement and establishing a continuum of care, increasing in-network utilization and the improvement of value based and publicly reported scoring.
Education REQUIRED:
High School Diploma Required.
Education PREFERRED:
Bachelor's Degree with a medical background is preferred.
Nursing, Social Work, Rehabilitation, Health Sciences is beneficial.
Experience REQUIRED:
A minimum of two years of clinical or healthcare environment experience.
Care Management Navigator must be an effective communicator with patients, families and various healthcare professionals.
Understanding of hospital based services and post-acute community resources.
Must be able to work independently and initiate processes when no direction is given.
Efficient at time management.
Project leadership and data analysis experience a plus.
Must have computer knowledge and the ability to work with multiple systems within the organization.
Knowledge of Managed Care, patient experience performance, quality measures and value based healthcare is preferred.
Must be emotionally mature and able to function effectively under stress.

Estimated Salary: $20 to $28 per hour based on qualifications.
