Case Manager (RN or LPN)
Days - 7on/7offAssesses and plans care that effectively manages a patient along the continuum in an efficient/cost effective manner while assisting in directing certain aspects of care from pre-admission (when appropriate) to post discharge.
Performs admission and continued stay review of patients, using Severity of Illness/Intensity of Service Criteria to determine appropriateness of admission, location/level of service (inpatient, observation, outpatient), and continued stays in the hospital.
Assesses and identifies patient's needs and assists patients/families/significant others with discharge planning.
Ensures that the outcomes of patient care are within the appropriate length of stay, that appropriate/effective resources are used, and that established standards are met through collaboration with the health care team and the patient/family/significant other.
Education REQUIRED:
Registered Nurse (RN) with current active nursing license in the state of Alabama required.
Excellent oral and written communication skills required.
Experience REQUIRED:
At least 3-5 years of nursing experience required.
Licenses/Certificates REQUIRED:
Current nursing license in the state of Alabama and in good standing with the Alabama Board of Nursing Recommended Skills Certified Nurse Practitioner Communication Diseases And Disorders Health Care Nursing Estimated Salary: $20 to $28 per hour based on qualifications.
